Rx Peps Direct, a Fort Collins, Colorado telehealth practice, charges a $39 fee for a provider’s review and has the pharmacy bill patients directly for medication. It has cared for more than 2,200 patients since May 2026.
FORT COLLINS, US — November 7, 2026 — FORT COLLINS, Colo., November 8th, 2026. Rx Peps Direct, a telehealth practice based in Fort Collins, has cared for more than 2,200 patients in 28 states and Washington, DC, since it took its first patients in May 2026. The practice splits every bill in two. Patients pay Rx Peps Direct a $39 medical review fee for a licensed provider to review their request. If the provider approves a prescription, a licensed U.S. pharmacy fills it, ships it, and bills the patient directly at the pharmacy’s own price. Rx Peps Direct adds nothing to the medication cost and earns nothing on it.

Many cash-pay telehealth services quote a single price that covers the provider’s time, the medication, and shipping. Founder and CEO Marcus Hansen said Rx Peps Direct started out that way and changed course after patients said they couldn’t tell what they were paying for.
“With one price, patients don’t know which part is the care and which part is the medicine,” Hansen said. “Most people already understand the other way from their own doctor. The doctor writes the prescription, and the pharmacy charges for the medicine. We set Rx Peps Direct up the same way, so a patient can see what they pay us for and what they pay the pharmacy for.”
Every price is listed on the Rx Peps Direct website before a patient creates an account. The process has four steps:
1. The patient picks what they want reviewed and sees the $39 fee and the pharmacy’s price up front. 2. The patient pays Rx Peps Direct $39 and completes a health history. 3. A licensed provider reviews the request and approves or declines it. 4. If the provider approves it, the pharmacy fills the prescription, ships it, and bills the patient directly.
Because the practice doesn’t earn anything on the medication, Hansen said, it has no reason to raise that part of the price. “Our fee pays for the part we actually do, which is the provider’s review,” he said. “The pharmacy sets its own price, and the patient pays it directly.”
In October, Rx Peps Direct added a second option for patients who want more time with a provider. The $99-a-month Concierge plan pairs members with one nurse practitioner for a 20-minute video visit every month, a check-in every two weeks, and unlimited messages, and it drops the $39 fee on their orders. The pharmacy still bills medication separately. The Concierge plan is available in 11 states where its nurse practitioner is licensed, and the pharmacy can ship.
Hansen’s advice for anyone comparing telehealth services is to ask one question before booking. “Ask whether the medication is included, and if it is, who set its price,” he said. “If the answer is one number, ask what the provider’s review costs on its own.”
