Designed to work alongside existing care teams, Ciba Health extends chronic disease management beyond the clinic for adults living with obesity, cardiometabolic diseases, digestive disorders, autoimmune conditions, and women’s health conditions. Ciba Health leverages artificial intelligence across risk stratification, care plan personalization, and ongoing patient monitoring, helping the multidisciplinary care team to identify emerging risks early and tailor interventions to each patient in real time.
Chronic disease requires continuous support, yet patients spend only a small fraction of their time with their healthcare providers. Between visits, gaps in care can contribute to worsening symptoms, reduced adherence, avoidable utilization, and disease progression, while providers face growing patient complexity, staffing shortages, and administrative demands.
Ciba Health addresses this gap by extending the ambulatory care team between visits through physician-led precision chronic disease management by finding the underlying drivers of symptoms.
